• Caring for families since 2004
  • A care manager on call, day or night
  • We never accept referral fees

Virtual care management gives your loved one and your family independence, with someone behind the scenes keeping their care on track.

A care manager works with you by phone and video. They handle the appointments, the paperwork and the calls with doctors and agencies, and make sure nothing slips through the cracks. That leaves you free to spend your time together doing what you enjoy.

It is the same team and the same standard of care our local families have, without anyone needing to move.

What your care manager does

  • Gets to know your loved one, their health, their home and what matters to them
  • Writes a care plan the whole family can see, and keeps it up to date
  • Joins appointments by phone or video, asks the questions and writes up what was said
  • Finds and checks home care agencies and other local services, then keeps an eye on them
  • Talks with doctors, pharmacies and agencies, so they have one person to call
  • Lets you know what has changed and what needs a decision

You stay informed, and you stay in charge.

If an in-person visit is ever needed, we will talk it through with you and look at the options together.

Who it helps

Perhaps one of these sounds familiar.

Moving is not on the table, and the main need is someone to keep the care organized.

You are close enough to visit, but the calls, forms and appointments have taken over. We take those on, so you can enjoy your time together.

Your loved one is in another state, and you are still the one handling every call. You need someone to keep watch and tell you what is really going on.

You live on your own and want a plan, and a person who knows you, in place before a crisis.

After a hospital stay or a move, the situation is steady and you need fewer visits, but still someone keeping track.

Is it as good as in person?

For many families, yes. Most of a care manager’s work is coordination: talking with doctors, agencies and family, keeping the plan current and acting quickly when something changes. That works well by phone and video.

When your loved one needs someone with them often, our in-person care management is the better fit. We will tell you honestly which one you need.
