Elder law, estate planning and trust work often depend on knowing how a client is doing day to day. A care manager can give you that picture. We assess your client’s needs, help put a care plan in place, and, with the client’s permission, keep you informed as things change.
Medical providers and discharge planners
A discharge or a new diagnosis often leaves families with more questions than your team has time to answer. We help the family understand their options, arrange care at home or a move, and follow through on the plan you set. A care manager is on call day or night, so the family has someone to call after they leave your care.
Senior living communities
A move is a big change for a family. We help them choose the right level of care, prepare for the move and settle in. Afterwards we stay involved and work with your team, so the family has one point of contact.
Home care agencies
Care management and home care work well together. We do not employ caregivers ourselves. Your caregivers handle the daily care, and we handle the planning, the medical coordination and the family communication, so your team is not caught in the middle.
