How we can help

A care manager becomes the one point of contact for family, doctors, the community and the insurer, all working from one plan.

We start with a private conversation about who is involved and what is stuck. Then we assess your loved one’s needs independently, so the family has a professional view to work from. We meet with the family, including for the hard conversations, so everyone hears the same recommendation at the same time. Afterwards, you get the plan in writing.

What that usually means in practice:

  • One written care plan that family, providers and the community all work from
  • Being the number everyone calls first, instead of you
  • Family meetings arranged and chaired, including the hard ones
  • Making sure the community hears what the specialist said
  • Decisions written down, so everyone remembers what was agreed
  • The same update reaching everyone who should have it, at the same time
  • A professional recommendation, so it is not one family member’s opinion against another’s

You do not need everyone to agree before you call. The first conversation is free, nothing is shared with the rest of your family, and there is nothing to sign.
