For the individual with dementia and their family, a diagnosis of dementia can upturn their lives. You might feel a sense of loss and a fear of the unknown. Family dynamics change, adult children become caregivers, and the family’s bandwidth is spent on managing and navigating care. A person living with dementia can still thrive, enjoy hobbies, and have quality time with loved ones, but this is difficult to do when the challenges of managing the diagnosis can be overwhelming. Our goal is two-fold: (1) bring together the resources to keep the individual with dementia safe and thriving, and (2) help the family manage the care through guidance, support, and advocacy so they can focus on spending quality time with their loved one.

To create a customized care plan, person-centered care focuses on the individual’s unique values, preferences, and identity. Our care managers have experience with many types of dementia, but that experience is only half of the equation. We also get to know our clients, their families, their values, and their unique physical and emotional needs.  It is this combination of knowledge about dementia and knowledge about the individual that creates person-centered care. For example, we know that dementia often causes an aversion to water and showers. But if we also know your loved one’s favorite scent and that they enjoy activities such as singing, we can turn a stressful shower experience into a meaningful experience like a spa sing-along.
