ElderTree Care Management Services was founded in 2004 by Kate Caldwell, MA, CMC on the philosophy that the elderly should lead the most fulfilling life possible with the upmost dignity focusing on their physical, mental, and spiritual strengths and well being. Kate has always had a deep passion for working with the elderly stemming all the way back to high school when she worked as an activity assistant in a nursing home. Kate’s passion for serving elders inspired her to found ElderTree and to put together a strong team of compassionate, certified, and professional care managers that share in her philosophy.
The ElderTree team is passionate about their work and dedicated to advocating for the elderly and disabled. We specialize in Care Management and recommend services that are appropriate for your loved one. ElderTree is a member of Aging Life Care™ (ALCA), previously known as the National Association of Professional Geriatric Care Managers. Our care managers are certified or in the process of being certified by the National Academy of Certified Care Managers (NACCM).

We are connected to the community
ElderTree is uniquely connected in the community. We know the local eldercare services and their reputation, and can help ensure success by working with others whose history and strengths we understand. ElderTree supports various local organizations including Insight Memory Care Center, formally known as Alzheimer’s Family Day Center, which is a non-profit organization providing a wide range of services to people with Alzheimer disease and their families. ElderTree also supports National Capital Area Alzheimer’s Association as well as Senior Services of Alexandria.
Our Code of Ethics
ElderTree Care Management Services abides by the code of ethics set by Aging Life Care™ as well as the Case Management Society of America. The Code is based on five core values for the ethical practice of geriatric care management:
